Additional Instructions for Clinical Instructors
After you have become familiar with STVHS policies and guidelines and completed the validation of the materials, the completed forms are to be delivered, faxed or scanned/emailed to:

Instructor of clinical rotations must also provide an alphabetized list of their students’ names.  All student paperwork should be compiled into one packet and arranged in alphabetical order by student.

In addition, each Clincial Instructor must complete the following orientation/education requirements:

• Nursing Orientation Class – All new clinical instructors must attend.  Please note that classes are scheduled two weeks in advance and to request dates/times/schedule contact  MaryAnn.Davis@stvhs.com.
• Horizon Electronic Documentation – Register and complete training to obtain user id/password.  This class is required for all new clinical instructors and preceptor students, and other students who will be using the documentation system.  (Instructor will be responsible for documenting all student documentation of interventions if students will not be using the system.)   Schedule this class by calling or emailing  Laurie O’Dell at 205.838.6729 or email Laurie.Odell@stvhs.com or Jennifer Helms at 205.838.6728 or email Jennifer.Helms@stvhs.com.

Evaluation of Clinical Experience
We value your feedback and strive to provide the best clinical learning opportunities possible. Upon completion of a group clinical experience, the instructor is kindly asked to complete and submit to Jessica Crews (by email, fax, or mail) the Student Nurse Instructor Evaluation of St. Vincent’s Clinical Experience.

Upon completion of a preceptorship, the student is kindly asked to complete and submit to Jessica Crews (by email, fax, or mail) the Student Nurse Preceptorship Evaluation.
