Welcome to the Ascension Patient Portal for your hospital-based health information. Your patient portal will include components of your medical record from St. Vincent’s hospital stays after December 2, 2017.
Directions to set up your Ascension Patient Portal
It only takes a few minutes and four simple steps to set up your personal patient portal, available anytime from your computer, tablet or mobile device!
Beginning December 2, 2017, there will be three ways to access your health information:
- Use the Ascension Patient Portal to access records for hospital stays on or after December 2, 2017
- Use Follow My Health to access records for hospital stays before December 2, 2017
- Use the St. Vincent’s Health System Portal to access records for Physician office visits at St. Vincent’s Family Care/Primary Care Centers
How to set up your new Ascension Patient Portal account
1. Beginning December 2, 2017, visit any registration desk to sign up.
2. You will immediately receive an email from Ascension Patient Portal <noreply@iqhealth.com>; click the secure link in the email.
3. Follow the online instructions to set up your username and password.
4. Verify your identity by entering your pin which is your four-digit birth year.
Once you have completed the first-time login, access your Ascension Patient Portal by visiting mystvincents.iqhealth.com/home
Follow My Health Patient Portal
If you had a St. Vincent’s hospital stay between March 12, 2014 and December 1, 2017, and you signed up for the Follow My Health Patient Portal, you can still access your information here. Your Patient Portal historic information from office visits, test results, etc. from March 12, 2014 to December 1, 2017 will still be available through the Follow My Health Portal.
